Lymphoepithelioma-like carcinoma is een zeldzaam carcinoom
van de huid, voor het eerst beschreven in 1988. Het is meestal een diagnose
die achteraf gesteld wordt door de patholoog op materiaal dat ingezonden wordt
onder de verdenking basaalcelcarcinoom of plaveiselcelcarcinoom. Meestal zijn
het vast aanvoelende plaques of nodi, huidkleurig tot erythemateus, in het gelaat
of de nek. Het komt voor op oudere leeftijd (rond de 70). Dit type carcinomen
kan behalve in de huid ook voorkomen in de nasopharynx, speekselklieren, longen,
thymus, maag, cervix en galblaas. De origine van de tumor is niet precies bekend,
mogelijk een adnextumor met eccriene of trichilemmale differentiatie. Voor
lymphoepithelioma-like carcinoma in de nasopharynx wordt nu een andere term
gebruikt, namelijk undifferentiated nasopharyngeal non-keratinizing
carcinoma (UNPC). Bij de
lokaties in de long, thymus, speekselklieren en de maag is er een associatie
met het
Epstein Barr virus (EBV).

PA: nesten of lobuli van epitheloide cellen, niet in
verbinding met de epidermis, in combinatie met een dicht infiltraat van T- en
B-lymfocyten. De cellen hebben een eosinofiel cytoplasma, prominente nuclei,
en er is een verhoogde celdeling met atypische mitose figuren. De lymfocyten
in lymphoepithelioma-like carcinoma kunnen soms CD30 positief zijn.
DD: basaalcelcarcinoom,
plaveiselcelcarcinoom, metastase,
metastase van een lymphoepithelioma-like carcinoma elders (vooral nasopharynx),
melanoma,
cutaan lymfoom,
lymphadenosis cutis benigna,
Merkel cell carcinoma.
Diagnostiek:Biopt, excisie en zonodig re-excisie. Beschouwen
als een plaveiselcelcarcinoom. Afbeeldend onderzoek (CT-scan hoofd en thorax),
consult KNO.
Therapie:
Excisie met
een marge conform stadium I laag risico plaveiselcelcarcinoom (0.5 cm).
Radiotherapie
(ook effectief en zeer geschikt voor moeilijke locaties of na niet radicale
excisies).
Prognose:De prognose is goed, na excisie
en of radiotherapie treedt meestal geen recidief op, en metastasering is zeldzaam.
